The Assisted Living Waiver (ALW) reimburses licensed RCFE and ARF providers using a tier‑based daily rate. Each resident is evaluated by a DHCS Nurse Evaluator and assigned a care tier, and the facility is paid per authorized day based on that tier.

2026 ALW Daily Rates

  • Tier 1: $95.69 per day
  • Tier 2: $114.33 per day
  • Tier 3: $132.97 per day
  • Tier 4: $179.58 per day
  • Tier 5: $270.80 per day

Additional ALW services—such as Residential Habilitation, Care Coordination, Transition Services, and Augmented Plan of Care—may be reimbursed separately when authorized.

Important: ALW does not pay for Room & Board. Residents pay their own rent, typically using SSI/SSP.

Leasing a home for the purpose of licensing it is far more complex than a standard rental.

Key Realities

Fire clearance is issued by the local Fire Authority Having Jurisdiction (FAHJ) and determines:

  • How many residents the home may serve
  • Whether non‑ambulatory or bedridden residents are allowed
  • Whether sprinklers or additional exits are required

What the Fire Marshal Evaluates

  • Mobility Levels — Ambulatory, non‑ambulatory, bedridden
  • Sprinkler Requirements — Needed for >6 beds or multiple bedridden residents
  • Egress Requirements — Clear exit paths, no steps
  • Room Size & Layout — Bedrooms must meet Title 22 (RCFEs) or Title 17 (ARFs)

  • Capacity Determination — Fire clearance sets the maximum licensed bed count
